ARISE KIDS CAMP

Mark your calendars for ARISE Kids Camp! For kids ages 6-13, it runs from July 21st through the 23rd. Drop-off begins at 8:30am and pickup begins at 1:00pm. The Cost is $40 per child and if a parent volunteers for the entire camp you get one child for free! Lunch is provided each day of the camp. We hope you can join us!

I hereby waive, release and discharge any and all claims for damages for personal injury which may hereafter occur to my child as a result of participating in said event. This release is intended to discharge in advance Revelation Church Orlando , its officials, officers, employees, volunteers and agents from liability. It is understood that some recreational activities involve an element of risk or danger of accidents, and knowing those risks, I hereby assume those risks. I give consent for my child/children listed above on this form to participate in all ARISE Kids Camp 2021 activities hosted by Revelation Church Orlando on July 21, 2021 through July 23, 2021 during the hours of 8:30am to 1:30pm. Activities include an outdoor obstacle course, water based games, crafts, as well as participation in other group activities. All while under adult supervision. I also give Revelation Church Orlando permission to publish in print, electronic or video format the image or video of my minor child. I release all claims against Revelation Church Orlando with respect to copyright ownership and publication including any claim for compensation related to the use of the materials. I hereby execute the above liability release and minor photo/video release on my listed children's behalf. Should I have any questions about any activities or do not wish for my child(ren) to participate in a specific event I will make this known to church staff and do so before signing this form. By providing consent below I declare that I am the parent/legal guardian of the child(ren) listed above and that all information entered is accurate.
